Tuition Fee's include: A Friendly Style Profile textbooklet, the Date' spiral workbook: Tools for Harmony Within Diversity, two customed-designed Personal Style Profile name cards, plus snacks and hot/cold drinks. Also included is the Friendly Style Profile inventory which is to be filled out in advance and returned to the Center for Dialogue and Resolution, CRES or Barbara ASAP and by Friday, March 20, 2018 at 2pm. Your inventory will be returned to you at the workshop. Surveys are available from CDR, CRES or Barbara.

NOTE: Your Registration will be complete when you return your Friendly Style Profile survey booklet with results to CDR, CRES or Barbara (drop off at UPS store at 2852 Willamette St. #256).

How to Register: There are three requirements that must be fulfilled in order to register.

#1: Complete registration form below and provide payment (see payment methods below).

#2: Pick up a Friendly Style Profile Inventory from CDR, CRES or Barbara, and fill out the questionnaire by 5pm on Wednesday, April 18, 2018. If you live outside of Eugene and wish to receive the inventory by mail, please select this option on the registration form below.

#3: Fill out and return the Friendly Style Profile Inventory with Self-Score sheet to CDR by Friday, April 20, 2018 by 2 pm. CDR is open Monday through Friday, from nine to five and is located at 93 Van Buren Street, Eugene OR, 97402. The completed booklet may also be turned into CRES or to Barbara (drop off at UPS store at 2852 Willamette St. #256).

Short Lunch Break: Bring your own or visit the nearby food court. 

Payment Methods: you may pay by cash, check or credit/debit card. Checks can be mailed to 93 Van Buren Street, Eugene, OR 97402. If you wish to pay by credit/debit card, you will have the opportunity once you submit your registration form.

Cancellation Policy: Full refund minus the cost of the materials ($32) if you cancel by April 20, 2018.  Fifty percent refund if you cancel after April 20, 2018 but on or before April 25, 2018. No refund if you cancel after April 25, 2018 or fail to be present on the day of training.

PRIVACY POLICY: Your privacy is important to us. We will never sell or otherwise provide your contact information to any non-related third party.
